Computer Science Educator of the Year Award Nomination Form

This award recognizes an Iowa Computer Science Educator who strives to achieve excellence through educating and informing students regarding the most up-to-date technologies being utilized by working professionals and innovators.

Instructions for completing the Iowa Computer Science Educator of the Year Application form: 

  • Read through the entire application before you begin.
  • Provide all information requested in the application form.  
  • Feel free to create your narrative responses on a separate document, then copy and paste them into this document.
  • Limit the length of your answers as requested in each section. Be sure to address all question components. 
  • In order to provide all applicants with an equal opportunity, no more than the maximum number of words requested will be accepted. Any additional pages and/or materials submitted will NOT be presented to the Selection Committee. 
  • Each application must include one photograph suitable for publicity purposes (5×7, color headshot preferred). 
  • The entire application package must be submitted by the nominated teacher.

    Maximum two double-spaced pages. 1) Beginning with the most recent, list colleges and universities attended including postgraduate studies. Indicate degrees earned and dates of attendance. 2) Beginning with the most recent, list teaching employment history indicating time period, grade level, and subject area. 3) Beginning with the most recent, list professional association memberships including information regarding offices held and other relevant activities. 4) Beginning with the most recent, list staff development leadership activity and leadership activity in the training of future teachers. 5) Beginning with the most recent, list awards and other recognition of your teaching.

    Describe a lesson or unit that you have delivered that defines you as a computer science educator. How did you engage all students in the learning and how did that learning influence your students? How are your beliefs about computer science demonstrated in this lesson or unit? (Maximum 750 words)
    Describe a project or initiative you have been involved in which contributed to the improvement of overall school computer science literacy.     How do you ensure that computer science education transcends the classroom? Describe specific ways in which you connect your students to their community. Please describe evidence of student impact. (Maximum 500 words)
    What do you consider to be a major issue in computer science education today? Describe how you demonstrate being a lifelong learner, a leader, and/or an innovator about this issue, both within and beyond the classroom walls. (Maximum 500 words)
